Usual inpatient dosing ranges for furosemide are 40–160 mg IV 1–3 times daily (maximum 200 mg per dose), or 5–20 mg/h by continuous infusion (maximum 40 mg/h), as outlined by the American College of Cardiology.[1-2] The DOSE trial demonstrated that higher initial dosing (2.5 times the home dose) resulted in greater fluid and weight loss, but ongoing assessment and titration are essential to avoid overtreatment and renal dysfunction.[3]
