Presentation

• Physical

  Centralized Back Pain OR Abdominal Pain

  • Can be in all quadrants or Diffused Abdominal Pain , typically Epigastric pain

  • Radiating to the back

  • Worsened by Alcohol (RF) or Food

  • Alleviated by leaning forward </aside>

  • Rebound tenderness

  • ↑ Tachycardia (>150), !SOB (Dyspnea), Hypotension ↓(<90/60)

  • Jaundice

  • Abdominal distension

  • +/- N/V - Nausea/vomitting

  • Ecchymoses (rare, signs of retroperitoneal bleeding)

    • Cullen’s sign
    • Grey Turner’s sign

  • Dullness at lung base (Pleural effusion)

Diagnosis

• BW:
  • Elevated WBC (3.5–10.5)
  • Elevated Liver Enzymes
• Lipase (or Amylase)
• Work up for Biliary Pancreatitis
  • US → MRCP (may find goal stone)
• Abdominal CT → Prognosis: Investigate necrosis of the pancreas

Treatment

• Mild: self-limited
• Moderate - severe: supportive care
