A/E

Common

• Fever (2-3 dys)
• Orthostatic hypotension (in titration)
• Salivation (Hypersialorrhea)
• Constipation

Rare but severe

• Agranulocytosis (first 4 to 6 months) & neutropenia (any time)
• Cardiomyopathy (late) & myocarditis (early)
• Bowel Obstruction
• Seizure (when > 500 mg daily) → Valproate, CI: G
  • Quit smoking = higher risk of seizures

Monitoring

Prior to Initiation

• H & P:
  • BP, HR, Orthostatic Vitals
  • BMI (18.5 - 25)
  • Waist Circum.(≤ 94/90 ♂, ≤ 80♀)
  • Hx of Smoking (RF)
    • *Smoking interact with Clozapine (↓ level)
    • Quit smoking = higher risk of seizures
