Based on the patient’s behaviour and questions around capacity today, we believe that [Patient] is able to understand, but not able to appreciate the consequences of medical decision involving [treatment]. We shared this finding with [pronoun_end], and noted that we will continue to involve [pronoun_end] in the discussion of treatment, but would be seeking formal consent from SDM, [SDM], for decision involving [treatment].
