Indications

Acute:

• PTX - Pneumothorax
  • Increased O2 in the alveoli -> Decreased FiN2 in the alveoli -> N2 in the tissue diffuse to the alveoli more rapidly -> N2 in the pleura diffuse to the tissue more rapidly -> decreased air in the pleural
• !Hypoxemic/Type I RF, Hypercapnia (Type II RF) (Respiratory Failure)
  • PaO2 < 60

Long term (home O2):

• PaO2 < 55 - must get oxygen at home long term
• PaO2 < 60 - indicated with other lung complications
  • Cor pulmonale
  • PH - Pulmonary hypertension
  • Erythrocytosis (indicate LT hypoxia adaptation)

Adverse Effects:

• Worsening hypercapnia through loss of hypoxic drive to breath (acute)
• Worsening V/Q mismatch
• Haldane effect: in the presence of oxygen, Hb more readily let go of CO2, leading to increased CO2 dissolved in the blood → Worsening hypercapnia
• Smoking while on oxygen - fire hazard
• Equipment - tripping hazard
