Pathophysiology

• Clonal stem cell disorder, often with JAK2, CALR, or MPL mutations.
• Excessive fibroblast stimulation leads to bone marrow fibrosis — the marrow literally turns to scar tissue.
• Hematopoiesis shifts to spleen and liver → massive splenomegaly.
