
Assess:
- Cardiovascular → Pediatric Vitals
- BP, HR, PP
- Skin appearance → General Inspection
- Colour (mottling/acrocyanosis), temperature, capillary Refill < 2s
- CNS → Modified GCS for Children
- Alertness, LOC
- Urine Output (Wet Diapers for NB ?)
- Weight loss → know Birth Weight (2.5 - 4.5), Pediatric Growth Assessment
- Physical Exam Finding → Routine Newborn Exam
- Mucous membranes: buccal mucosa etc.
- Fontanelle Exam → Anterior fontanelle (typically closes before age 2 y.o.)
- Eyes (normal, sunken, or markedly sunken)
- Skin turgor (normal, decr, or tentin)
