- ASIS on the table
- Arm straight → use arm to push the body up
- Make sure gluteus muscles are not engaged
- Add a pillow underneath the feet
- Internally rotate the feet

Performed when suspecting Pattern 1 - FA
- Prone Extension Positive (PEP) → No Pain
- Pain is reduced within 10 repetitions.
- There is an easily established “directional preference”, which indicates a patient who should respond rapidly with minimal intervention.
- Prone Extension Negative (PEN) → Pain
- Pain is either unchanged or increased within 10 repetitions.
- There is no obvious “directional preference” and treatment will probably require more specificity and care.
