RV polarize via LV myocardium conduction

1. Normal Axis or Right Axis
2. Left Axis = Bi-fascicular (WORSE Conduction)

Signs:

• Wide QRS complex (> 120 ms)

• V1, V2: “bunny ears” or “M” (rSR’) ”rabbit ear” for RBBB

  

• Lateral leads (I, avL, V5, V6): large downward deflection (S waves) or “N” (S)

  • S waves > 40 ms

  

• Can see T wave inversion in leads with large positive QRS deflection

ECG:
