Additional Presentation

RF

• Stopping antithyroid meds or beta-blockers
• Thyroid manipulation (thyroidectomy, neck surgery)
• Iodinated contrast exposure
• Recent radioactive iodine
• Sepsis, pain, trauma, major surgery (stress trigger) </aside>

PP/PN (Pathophysiology / Presentation)

POD 0–3 (sometimes up to POD 5 with infection).

• High fever
• Severe tachycardia ± AF
• Agitation → delirium → coma
• GI upset (vomiting/diarrhea)
• ↑ Hyperglycemia (> 7) , Lactic Acidosis
• Cardiovascular collapse (high-output failure → shock)

Diagnosis Criteria (Practical)

Clinical diagnosis.

Think storm when:

• Fever + tachycardia out of proportion
• CNS changes
• GI/hepatic dysfunction
• Clear precipitating event

Burch–Wartofsky >45 supports diagnosis.
