RF
- Stopping antithyroid meds or beta-blockers
- Thyroid manipulation (thyroidectomy, neck surgery)
- Iodinated contrast exposure
- Recent radioactive iodine
- Sepsis, pain, trauma, major surgery (stress trigger)

PP/PN (Pathophysiology / Presentation)
POD 0–3 (sometimes up to POD 5 with infection).
- High fever
- Severe tachycardia ± AF
- Agitation → delirium → coma
- GI upset (vomiting/diarrhea)
- ↑ Hyperglycemia (> 7) , Lactic Acidosis
- Cardiovascular collapse (high-output failure → shock)
Diagnosis Criteria (Practical)
Clinical diagnosis.
Think storm when:
- Fever + tachycardia out of proportion
- CNS changes
- GI/hepatic dysfunction
- Clear precipitating event
Burch–Wartofsky >45 supports diagnosis.
