Medial shift of uncus (medial temporal lobe) below the tentorium

• Compress ocular motor nerve (+/- cerebral peduncle)
• Seen with Midline shift
• 20-30 min to act before permanent damage

Presentation

• Triad of
  • CN III Oculomotor palsy involving pupil
  • Decreased LOC
  • Hemiparesis
