Approach to Vision Loss

With or without RAPD?

• RAPD → Neuorlogical
• non RAPD → Oculars
  • cornea, lens, vitreous opacities, refractive errors
  • Non-organic unilateral visual loss

Monocular vs binocular vision loss

• Binocular: retro-chiasmal
  • with reading impairment
  • visual acuity will be preserved

Any other focal neurological deficits?

Time course (Onset)

• < Days: vascular
• Days to weeks: Infectious or inflammatory
• Weeks to month: neurodegenerative

Pain with eye movement?

• Pain = inflammatory = Optic neuritis
• Yes – think Optic Neuritis (don’t often see optic disc edema cuz it can occur at the proximal optic nerve)

Fonduscopy:

• paler is more of an optic neuritis than NAION
